Optimal Health Coordinator
The Optimal Health Coordinator is responsible for developing, implementing, and evaluating educational lessons focused on healthy relationships, nutritional health, fitness, and overall wellness. This role involves collaborating with district and community stakeholders to design and deliver evidence-based practices aimed at increasing proactive health behaviors and reducing risk factors associated with unhealthy behaviors. These partnerships are built on respectful collaboration and data-driven dialogue.

Requisitos​
Bachelor's Degree in Public Health, Public Administration, Education, Social Work, Human Services, or a related field (Master's degree preferred).Â
Equivalent life experience considered.Â

Tareas y responsabilidades laborales
Responsible for all aspects of planning and implementing educational events and activities related to improving youth wellness at assigned building/district in partnership with the Community Schools model. Deliverables will be planned and implemented under the direction of the Community Schools Optimal Health Director, the Building Principal, and district leadership at each site.
Participate as a member of the school intervention team(s) responsible for the implementation and management of Multi-Tiered Systems of Support (MTSS).
Support the MTSS process of reviewing and analyzing student data to define the target population for Tier II and III interventions.
Help create and organize educational programs for your assigned site that align with the goals of improving youth health through the Community School Initiative and support the School Improvement Plan.
Work collaboratively with other health education partners to ensure cultural and linguistic appropriateness of the grant services, programs, and communication efforts.
Serve as a point person for agencies and programs interested in partnering with the building in support of health education activities.
Build relationships and mechanisms to effectively link youth participants to youth development programs with expanded learning opportunities/out-of-school time opportunities related to improved health and wellness.
Support the needs of the Get Ready to Grow Program
Support documentation and evaluation of the health educational program activities by coordinating the collection of data, timely submission of reports, and responses to other requests for information.
Participate in capacity-building activities, including initiative-wide and site-based trainings, and Wayne County Consortium network meetings.
Represent the community school strategy in various public forums as needed and participate in advocacy activities to promote the community school strategy.
Coordinate the allocation of space for programming and events.
Work Environment
Full-time, salary position with benefits.
The position requires a NYS Driver’s License and the ability to easily travel between designated program and office sites across Wayne County.
The position is expected to require evening and weekend availability to conduct programming, meet with youth and parent/guardians, and community stakeholders outside of the regular school day.
